Hospice By the Bay, the nation's second oldest hospice, is currently seeking a MSW or LCSW to be responsible as a member of the transdisciplinary team for providing supportive services to help meet patient/family social, emotional and financial needs related to the impact of a terminal illness, loss and grief. This position is based in Larkspur. Possible travel to Sonoma, may be required.

The ideal candidate will possess a demonstrated ability to effectively work with patients/families and the community, dealing with life-threatening illness and death; along with a strong working knowledge of social work practices.

Requirements include a Master's degree in Social Work from a school accredited by the Council on Social Work Education. The Nurse Practitioner (NP) is expected to work with minimal supervision, and to consult with the Medical Director and the Consulting Psychiatrist. The NP works closely with other clinical and medical staff to ensure that the medical services provided to our clientele are consistently adherent to the standards of the agency. The NP will work primarily within the Transitional residential programs.


1. Conducts health assessments and in some cases medical examinations for clients residing in Baker Places' residential programs. Reviews medications and clients' access to and utilization of primary care providers in the community and facilitates these linkages. Liaisons with primary care providers to discuss health and medication concerns.

2. Conducts routine review of diagnostic and psych assessments performed by external providers. This shall consist of reviewing the history of presenting circumstances, current medications, psychiatric history, five-axis diagnosis and a medication plan. Liaisons with community psychiatric providers to develop the most appropriate medication regimen given the client's history and presenting symptoms.

3. Works closely with the client and program staff to help enhance medication adherence through client staff education, one to one and/or group interventions with the client.

4. Monitors medication procedures and practices for quality assurance and compliance.

5. Provides program-based and/or agency-wide inservice training on various topics related to the provision of medical and psychiatric services to our target population.

6. Provides support and guidance to nursing program student interns when on site.

7. Other related duties and responsibilities as assigned.


Must hold current CA license and Board certification. Adult Psychiatric Nurse Practitioner preferred. Valid CA driver's license and willingness to travel between/among several sites throughout San Francisco. Experience working with community-based organizations and providing services to clients of the Public Health system who have co-occurring psychiatric and substance abuse disorders, often complicated by homelessness and HIV or other chronic conditions.
